I haven't seen it yet, but I've seen another meta way of circumventing star applause, which ties in to the posts above about Maggie Smith. In Lady from Dubuque Irene worth entered with a finger over her lips, warning the audience to ignore her. She continued to shush any applause. It was a brilliant stroke, the synergy of star and character; the titular woman - from a more potent place than Iowa - wasn't supposed to be noticed. The play, alas, didn't earn it.
